INTERVIEWS
An interview with parents and children to be cared for is required before any child will be accepted at Pam's Play Place. Interviews are normally conducted after hours. Parent may then schedule an appointment to visit to observe after the initial visit.

MEDICAL REQUIREMENTS
I comply with all state, county, and local regulations regarding immunization and health requirements. Infant, toddler, and preschool children’s immunization records must be kept current. It is extremely important for parents to advise me if a child has a health problem or allergies. A child with serious health problem must have an individual health plan, written by the nurse, in place before attending child care. (Examples include diabetes, seizures, etc.)

IMMUNIZATIONS
All immunizations must be kept up to date. Any child found not up to date will have 24 hours notice to receive required immunizations. The provider can refuse to care for any child/children if required immunizations have not been received.

Children may not return to the child care for at least twelve (12) hours after receiving immunizations or boosters. Parents are responsible for supplying provider with written statement from the doctor regarding current immunization records each time their child is inoculated.

Parents are responsible for requesting from provider a copy of the child care immunization card to take to the annual doctor visit and have updated. Your physician can also provide parents with the necessary card for your child’s required child care immunization records.

YEARLY PHYSICALS
Colorado Department of Health and Colorado Department of Human Resources require all children in child care to have annual physicals until the age of (8) eight. I will provide you with a “Health History” form to be completed by your child’s physician and returned to me. Please keep in mind I must follow strict guidelines regarding this ,matter and I am required to refuse admission to any child who does not have a current “Health Status Rep
ort” and “Certification of Immunizationon file.

SUPPLIES
Parents are responsible for supplying diapers, wipes, a full change of clothing, and any other supplies your child may need. I ask that parents bring a jumbo package of diapers and refill package of wipes (per child) at the beginning of each month. This should last your child for the entire month. All parents who have children who are not potty trained will need to supply wipes. If it becomes necessary for me to supply wipes or diapers for your child, reimbursement will be expected no later then your next payment date. Please have your child bathed and dressed for play. A clean child is a healthy child. I teach the children creativity, problem solving, and pride in their skills. Sometimes these values get messy. We will be painting every Tuesday so please send your child in old clothing on those days. Please dress your child for real success. If there is an occasion that calls for special clothing, please bring this clothing in an extra bag and I will help your child change before you arrive for pickup. Provider is not responsible for your child’s clothing or personal items brought into childcare home. I will let you know when I need more supplies for you child.

TUITION
All child care tuition is to be paid in advance. Tuition for full or part time contracted child care may be paid by a personal check from a local bank. Out of state checks will not be accepted. A convenient pay schedule will be arranged with each family. This schedule will be part of each individual contract. A minimum of thirty days notice is required when requesting a change in the contracted payment schedule. Payment is always due in advance on Friday afternoon (not Monday morning). There will be a $10.00 per day late fee for any payment received after this time. The late fee may be waived at the discretion of the provider. If a child is going to be on vacation or using a personal day, tuition will be due prior to the start of vacation or the payday missed. A fee of $10.00 per day will be charged for tuition received late. Provider may request tuition payment early when the normal payday is a legal holiday, personal day or vacation day for the provider.

Parent(s) are welcome to post date the early payment for the actual date due. Please refer to the financial policy of the child care home.

MULTIPLE CHILD DISCOUNTS
When multiple children from the same family attend Pam’s Play Place, a discount from the usual tuition fee is typically granted once a second child begins attending. To take advantage of this discount, full tuition must be paid for the highest tuition base. The discount is then applied to the other child enrolled from the same family.

ENROLLING YOUR CHILD
All child care available requires an interview with the parent(s) or legal guardian(s) of the child. All child care forms including the immunization record must be returned a minimum of 24 hours prior to the first day of enrollment. This allows the provider time to review the child care forms.

THE FIRST FEW WEEKS

The first few weeks in any new environment can produce anxiety for your child. I am sensitive to these feelings and I will make every effort to help your child adjust as easily as possible.

These are some things you can do as a parent to help ease the adjustment period:

  • Since your feelings may be indirectly communicated to your child, it is important that you feel confident in my ability and comfortable about bringing your child to my home.

  • If possible, plan to spend an hour or two visiting with your child on or before your child’s first day.

  • Talk with your child positively about child care and the things he/she will be doing there.

  • If you anticipate a problem in separation, discuss this with me and decide on a procedure to follow in advance. I suggest that after the necessary signing-in and exchanging greetings, you say to your child, “Goodbye. I will pick you up later. I know you will have a good day.” and then leave. This method may seem abrupt, but it will minimize separation anxiety for your child. This is preferred to “Sneaking out,” because your child is well informed and has the opportunity to say goodbye to you.

  • For younger children especially, it may be helpful to bring a special object from home, such as a stuffed toy or blanket. This may help the child in bridging the gap from the familiar and the unfamiliar.

  • During the first few weeks, you may feel some apprehension after leaving your child. This is a perfectly natural response – I know how very important your child’s well-being is to you. I encourage you to call me during the day to find out how your child is doing.

  • A readjustment may occur after a long weekend or vacation. Your continuing positive attitude will facilitate any such transitions.

REGISTRATION AND RENEWAL
There will be a registration fee in the amount of $50.00 payable with the deposit upon enrollment. There is also a $35.00 Renewal fee due annually with your first tuition payment of the New Year. These fees help cover any supplies which need to be purchased, such as crayons, paper, paint, glue, etc...

ASSISTANCE IN PAYMENT
Parents or Guardians receiving assistance in payment of child care tuition or fees from any source are still legally liable for all tuition and fees due to Pam’s Play Place according to contract in the event of a non-payment or insufficient funds etc…

RETURNED CHECKS
There will be a $30.00 cash fee for any returned check, plus the amount of the check in cash.

Parents will be charged for all fees incurred by provider as the result of the returned check. This includes Any bad checks the provider may write in good faith not knowing a tuition check was returned. Parents will be placed on a cash only basis once this situation arises.

RECEIPTS & W-10 FORMS
I will supply a written receipt for each payment made, if you request one. I will give you a monthly statement with payment made and balance due. I will also supply you with a W-10 so that you may claim your childcare credit on your taxes. This W-10 will be given to you in January of the New Year or when services are terminated. It is imperative that both parties claim the same amount on their taxes as this will send up a flag to the IRS and may result in an audit of both parties. If you think that the amount is incorrect then you must let me know so that it can be researched and corrected if needed.

RENEWAL OF FORMS
All forms and contracts will be renewed annually on June 1. All previous forms are obsolete.
