Permission to Participate in Child Care Activities
I/We hereby give Pamela Ball permission to take my/our child, , off the premises and on excursions that will take place during regular childcare hours. I/We understand these trips may be made by foot, in a reliable vehicle, or public transportation. I/We understand that these trips will be supervised by a responsible adult and that all precautions will be made for the safety and well being of all the children. I/We also understand that Pamela Ball will not be liable for any accident or injury to my/our child .

I/We ________________________________ hereby give Pamela Ball permission to allow my/our child to use all indoor and outdoor play equipment and to participate in all the activities of Pam’s Play Place and Preschool.

I/We ________________________________ hereby give Pamela Ball permission for my/our child to participate in all field trips away from the premises of Pam’s Play Place and Preschool under the supervision of a responsible adult.

I/We ________________________________ understand that I/we may be responsible for field trip expenses and we agree to accept this responsibility as it is stated in Pam’s Play Place and Preschool Parent Handbook.

I/We understand I/we are responsible for my/our child’s fees, when necessary. I/We understand it is not necessary that my/our child attend field trips but I/we will be required to find alternate care for my/our child if he/she does not attend. Regular childcare fees will still apply for field trip days, whether or not my/our child attends. I/We also understand that I/we may be asked to leave my/our child’s safety-approved car seat for Pamela Ball to use for my/our child during any field trip.
